POLICIES
ADMISSION
POLICY
It
is policy of each facility to admit and to treat all patients
without regard to race, color, national origin, creed,
sex, religion, age or handicap. The same requirements
for admission are applied to all, and patients are assigned
within the home without regard to race, color, national
origin or handicap.
At
our facilities, all individuals are granted an equal opportunity
for employment, training, promotions, and benefits, without
regard to race, color, creed, religion, sex, national
origin, age, or handicap.
The
resident is fully informed, as evidenced by the resident’s
and/or responsible party’s written acknowledgement, prior
to or at the time of admission; and during stay, of these
rights, and is given a statement of the facilities responsibility
to obey all reasonable regulations, and to respect the
personal rights and private property of other residents.
The State requires a signed agreement by the resident
and/or responsible party, to become a part of the resident’s
medical record.

1:
FIRE DRILLS are conducted every month and it is
mandatory that all residents participate in all fire
drills.

SAFETY REGULATIONS have been developed by our professional
personnel. All residents, visitors and the general public
shall be required to observe the safety regulations
for each facility. Residents and/or visitors will not
be permitted in the following areas:

Maintenance
Areas
-
Furnace Areas
-
Kitchen
-
Storerooms
-
Housekeeping Areas
-
Medicine Room (unattended)
-
Beauty Shoppe (after hours)
-
Employees’
Lounge
-
Inside Nurses’ station (unattended)
-
Treatment Rooms (unattended)
-
Physical Therapy (unattended)

3: CIGARETTE LIGHTERS AND MATCHES are not allowed
in a resident’s possession. Smoking is permitted in
solariums and lounge areas only. No smoking is permitted
in the resident’s room at any time, by residents or
visitors. THIS IS A STATE LAW. We also ask that you
do not give cigarettes, etc. to other residents, as
many residents should not smoke.

FOOD may be brought in after checking first with
the nurses on duty. Please do not leave food in the
rooms. It may be left at the Nurses station and we will
see that it is given as desired. Also, we ask that you
do not give food or candy to other residents without
checking with the Nurse, as many residents are on special
diets. All food must be kept in a closed container.

5: VALUABLES: It is generally recommended that
valuables, expensive jewelry, money, checkbooks, blank
checks, important papers, etc. not be kept in the resident’s
room. The nursing home will not assume responsibility
for any losses. The resident’s and/or responsible party
must assume responsibility for the storage of any personal
items (excess clothing, luggage, etc.).

MEDICAL EMERGENCY: In the event nursing service
is unable to reach your private physician with in four
(4) hours, or in an emergency, the facilities’ Medical
Director will be called for emergency orders and his
orders will be followed (Example: Send to Hospital).
The resident and/or responsible party will be consulted
on move if there is one, but, in all events, will be
informed of what action is taken. In the event nursing
service is unable to contact any physician or responsible
party, the charge nurse will send the resident to the
hospital.

7. WITNESSING LEGAL DOCUMENTS: Only notary publics
shall be permitted to witness legal documents. Please
notify the Admissions Director when needed.

YEARLY PHYSICALS AND LAB TESTS: All residents are
required to have a physical and history by the physician
on admission, and annually thereafter.

SIGN IN/OUT: Because many of our residents are confused
at times, we find it necessary to know at all times
where each resident is. For this reason, it is necessary
for each resident and/or responsible party to sign out
before leaving with a resident back in. Please see Charge
Nurse to sign residents out and in.

10. VISITORS must observe visiting policy.

11. LEVEL OF CARE: The level of care ordered
by the attending physician dictates the location and
type of room to be assigned, depending upon the professional
care required. A resident may be transferred to another
location in the nursing home should his/her level of
care change. Residents will be consulted in this decision
and administration will give every possible consideration
to resident’s desires, if and when a transfer becomes
necessary.

CONCEALED FACTS: Residents and/or responsible parties
who misrepresent or conceal facts relating to eligibility
for admission before or at the time of admission may
cause the resident to be discharged, if admitted, or
the admission denied.

COMMUNICABLE DISEASE: Any resident with a known
communicable disease will be placed in isolation with
isolation techniques being observed in his/her care
and treatment.

CHARGES: Each resident is fully informed and is
given a written statement prior to or at the time of
admission and during stay, of services available, and
of related charges, including any charges for services
not covered by sources of third-party payments or not
covered by the facilities’ basic per diem (daily) rate.
Our
facilities limits access to any personal, medical and
social records to those people directly involved in
the resident’s care. Only personnel concerned with the
fiscal affairs of the resident have access to the financial
records.
In case of transfer or discharge to another Health Care
Facility, resident’s medical information will be outlined
on a standard transfer form and sent with the resident.
Our
facilities will, at all times, provide privacy when
residents are dressing, bathing, etc. A closed door
or a drawn curtain will be used to shield the residents
from passers-by. People not involved in the care of
the residents are not to be present while the resident
is being examined or cared for.
The
residents are not required to perform services that
are not for the therapeutic purposes in his/her Plan
of Care.
Visiting
is suggested between 11:00 a.m. and 8:00 p.m. Members
of the clergy are welcome at any hour.
Subject
to reasonable scheduling restrictions, visiting policies
and procedures permit residents to receive visits from
any one they wish. A visitor may be restricted by the
facility for one of the following reasons:

The resident refuses to see the visitor.

The resident’s attending physician documents specific
reasons why such a visit would be harmful to the
resident’s health.

The
visitor’s behavior is unreasonably disruptive. This
judgment must be made by Administration and the
reasons documented. This will be kept on file.
Resident
lounges and recreational areas are also provided for residents
to receive visitors in reasonable comfort and privacy.
